When several health concerns overlap, a separate appointment for each symptom may leave families without a clear overall plan. A comprehensive geriatric assessment brings the older person’s medical and everyday priorities into one consultation pathway.
A review built around the older person
At Jeevan-Prerana Senior Citizens’ Clinic, the starting point is what has changed and what matters to the patient. The assessment can consider medical conditions alongside medicines, memory, mood, movement, nutrition, continence, vision, hearing and daily activities. Family observations can help when the patient wishes a caregiver to participate.
- Changes in walking, strength, appetite or ability to manage at home
- Several conditions or medicines that need a coordinated review
- A new care plan after illness or hospitalization
- Questions about prevention, support and maintaining independence
What to bring to the appointment
Bring current prescriptions or medicine containers, recent test reports and discharge summaries. A short list of changes noticed at home, previous falls and the patient’s own priorities helps focus the discussion. A family member may accompany the patient with their agreement.
What happens after the assessment
The physician discusses the main concerns and agrees on next steps. These may include follow-up, an investigation to answer a specific clinical question, a medicine review or support from another professional. Every patient does not need every test or allied-health service. The clinical findings determine the plan.
Assessment and clinic packages
A physician consultation is the starting point. The clinic also describes its Dirghayu options on the booking page. Please check the current inclusions and fees before payment; an assessment does not mean that all investigations are included.
